Transpericardial phrenicotomy from a dagger wound.

چکیده
Many examples of penetrating wounds of the heart and pericardium have been recorded (Barrett, 1950). The phrenic nerve has occasionally been divided in the neck as a result of knifing (Reichl, 1932) or of gun-shot wounds (Neuhofer, 1922), but the literature contains no reports of injuries to the phrenic nerve in the chest in which the patient has survived. The following case is therefore of interest. Two months later there was some tone in the diaphragm and the lung had re-expanded (Fig. 3). By March, 1953, 11 months after the injury, the right diaphragm was moving normally but had not quite recovered its full excursion. In the meanwhile the patient had become pregnant and was normally delivered. Electrocardiograms 36 hours after admission (Fig. 4a) showed elevation of the S-T segment in all chest leads and an inverted T-wave in lead 3. A week later (Fig. 4b) the T-wave was inverted in leads V2 and V3, the leads over the right ventricle, but the S-T segment was now normal. After two weeks (Fig. J4c) the E.C.G. had reverted to normal, T3 now being upright. The QRS type of VI lead and the QR complex in AVR indicate an unduly clockwise rotated heart with the right ventricle more anterior than usual.

Contribution of the spontaneous crossed-phrenic phenomenon to inspiratory tidal volume in spontaneously breathing rats.
Spinal cord hemisection at C2 (C2HS) severs bulbospinal inputs to ipsilateral phrenic motoneurons causing transient hemidiaphragm paralysis. The spontaneous crossed-phrenic phenomenon (sCPP) describes the spontaneous recovery of ipsilateral phrenic bursting following C2HS. We reasoned that the immediate (next breath) changes in tidal volume (V(T)) induced by ipsilateral phrenicotomy during spon...
متن کاملEffects of phrenicotomy and exercise on hypoxia-induced changes in phrenic motor output.
To investigate models of plasticity in respiratory motor output, we determined the effects of chronic unilateral phrenicotomy and/or exercise on time-dependent responses to episodic hypoxia in the contralateral phrenic nerve. Anesthetized (urethane), ventilated, and vagotomized rats were presented with three, 5-min episodes of isocapnic hypoxia (11% O(2)), separated by 5 min of hyperoxia (50% O...
متن کاملTranssternal transpericardial operations in the treatment of bronchopleural fistulas after pneumonectomy.
Between 1972 and 1993, 19 patients (15 males and 4 females) with bronchopleural fistulae and pleural empyema after pneumonectomy were treated with transsternal transpericardial operations and closure of the fistula. The underlying malignant disease was a non-small cell carcinoma in 12, a malignant epithelial mesothelioma in two, and an atypical carcinoid tumor in one case. One patient each pres...
متن کاملPericardial influences on ventricular filling in the conscious dog. An analysis based on pericardial pressure.
Twenty-five dogs were chronically instrumented to investigate the effects of the normal pericardium on cardiac function. Pulse-transit ultrasonic transducers were implanted to measure multiple ventricular dimensions. The pericardium was incised transversely at the base of the heart and precisely reapproximated, so as to disturb its characteristics minimally. One week later, the dogs were studie...
متن کاملThermal inactivation studies of normal and variant human erythrocyte carbonic anhydrases by using a sulphonamide-binding assay.
Heat-inactivation studies were carried out on the two primary erythrocyte carbonic anhydrase isoenzymes, CA I and CA II, and the secondary isoenzyme of CA I, CA I (+1). In addition, two genetic variants of human isoenzyme CA I, CA Id Michigan (100 Thr-->Lys) and CA If London (102 Glu-->Lys), and one variant of isoenzyme CA II, CA IIh (251 Asn-->Asp), were similarly analysed. The first-order rat...
